- As a social assistant at the HUGs, you are being shown to the patient in the Geriatric and Rehabilitation Department in a comprehensive and multidisciplinary approach
- You are ensuring the social follow-up of hospitalized patients and patients, particularly in an acute care unit
- Assessing the social needs of patients and their families, identifying social difficulties and proposing appropriate solutions
- Intervening in the management of administrative and financial demands, and assisting patients in their efforts to maintain the health of their patients and their families.
- Work closely with the in-house multidisciplinary team and external partners - Conduct individual, family and multidisciplinary conferences to build trust, understand patient situation and develop concrete and customized solutions - Ensure rigorous transmission of patient and patient needs and projects to the care team and partners
- Actively participate in professional monitoring on social and legislative issues, in particular social insurance, and in the prevention of social problems - Be involved or involved in the continuous improvement of practices within the service
