- Mediation and organisation of medically prescribed rehabilitation and spa stays
Main responsibilities
- Counselling, care and support of patients and their relatives in sometimes difficult life situations
- Mediation and organisation of medically prescribed rehabilitation and spa stays
- Support, advice and organisation of transfers to retirement and nursing homes
- Advice and support in the planning of ambulatory care at home (e.g.
- Spitex, meal services, etc.)
- Placement and advice on social, addiction and disability facilities and services
- Coordination with relevant external specialist bodies, authorities and municipalities
- Participation in interdisciplinary reports, visits, working groups and projects
- Planning and organisation of patient transport
