Main responsibilities
- The advice and care of the inpatient patients and their relatives is taken over in cooperation with the specialist centres involved
Company and context
- Coordinate the exchange of information with specialists from pre- and post-treatment organizations as well as when taking over and handing over the case
- You work closely together in the interdisciplinary treatment team
- The placement and organization of nursing institutions and counselling centres as well as the clarification and advice on insurance and financial issues you take on independently
- With targeted help for self-help, you support patients in social matters (social insurance, finance, work, etc.)
- The organization of the financial affairs of persons with adult protection measures is carried out in cooperation with the competent authorities
- In special situations, you advise and accompany patients and their relatives psychosocially Requirements
- You have an education in social work at tertiary level (HF, FH or a similar degree recognized by decree in Switzerland)
- With the different needs of our patients and the interdisciplinary treatment team you deal professionally and appreciatively
- As a committed and resilient team player, you are characterized by high flexibility and willingness to change A quick comprehension and organizational skill are among your strengths
