- Communication with patients and their relatives in connection with the planning/organisation of follow-up solutions after the hospital stay
- Cooperation with Spitex organizations, rehabilitation clinics, retirement and nursing homes and other institutions to control and organize targeted follow-up care for patients
- Obtaining cost language for stationary aftercare
- Support of the support teams to maintain the optimal dwell time by early clarification and control of the stationary course and exit planning
- Interdisciplinary cooperation with the nursing stations, nursing experts, doctors and clinic secretariats
