Care professional HF/FH 80-100 % (m/f/d)

In the offer "Care for relatives" clients in need of care receive the best possible person-centered care by their relatives. The caring relatives are advised, accompanied and supported by care professionals HF/FH. For the work done, the caring relatives are honored and insured. By agreement we are looking for a nurse HF/FH with case management for the Central Switzerland region.

Their tasks

• You lead the care process and ensure the best possible person-centered care quality for clients
• They provide guidance and support to the family.
• They organise relief offers
• They help to expand the offer
• They like to work independently, are well organised and act responsibly
• As a committed nurse, you have strong communication skills
• For this responsible task, we require an HF/FH degree and nursing experience, preferably in Spitex.
• You have a valid driving license and your own car
