To strengthen our newly created dementia team in Reinach we are looking for you as a Mansioni day service from Monday to Friday incl.

Public holidays

Self-employed, competent and resource-oriented care of our clients and their relatives in dealing with dementia diseases in their usual living environment
Take over of case management for a defined client group
Planning, control and monitoring of the care process
Support of our clients in a self-determined everyday life at home by promoting independence and quality of life
Interdisciplinary cooperation with relatives, doctors and other services
Conduct of the 5,7s and site discussions with RAI-HC
Requisiti
Completed training as a qualified nursing specialist HF
Professional experience and broad expertise
Supplementary training in dementia of benefit (or willingness to train in this field)
A self-employed, responsible working method
High empathy and empathy
Ready to be on the road with a car
