Main responsibilities
- Seeking out acute psychiatric care of patients
- Visiting and integrative case management of mentally ill patients before, during or after an inpatient stay incl.
- Case documentation and writing of reports for insurance
- Reference work involving nursing diagnostics
- Interprofessional cooperation throughout the treatment team
- Active participation and implementation of the project Hometreatment as well as in the further development of the offer Profile Completed HF or FH studies
- Professional experience in acute psychiatry for adults, experience in seeking care is desirable
- Readiness for teamwork, visiting care and the associated travel activity as well as flexible service planning (shift operation) Responsible and initiative working method Good knowledge of German and IT Foreign diplomas require SRC recognition Ticket Cat B Offer Experienced and motivated team
