- Carrying out care activities
- Planning and support of medical visits
- Electronic care documentation through care coach
- Taking on responsibilities as a reference person
- Control of the care process
- Management of case meetings
- Assumption of daily responsibility with coordination of the day team
- Assumption of tasks as a topic carrier, technical authority
Your profile
- Completed training for/for dipl. HF/FH/DNII nurse or equivalent training recognised by the SRC
- Professional experience in long-term care and/or in coordinated care assignments
- RAI knowledge (InterRAI-HC) desired or willingness to attend these courses
- A warm and humorous basic attitude
- Enjoying multiple challenges
- Proper knowledge of German, differentiated verbal and written expressive ability
