Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our offer ranges from counselling for older people and their relatives to general and specialized long-term care and care at home. As the largest public Spitex in the Dielsdorf district, we provide ambulatory services for our nine contract municipalities. With our comprehensive range of care services, we enable people to stay in their usual home for as long as possible despite health restrictions. In the future, it is planned to establish a night spitex.
Tasks: • Care, support and advice of our clients at home • Ensuring the care process • Case management and implementation of needs assessments in tandem with a dipl. Nursing professional • Preparation, adaptation and evaluation of care documentation • Consultation services • Correct collection of performance data • Cooperation with relatives, doctors, hospitals, homes and other specialist centres • Integrated 6-month internship in inpatient acute care and transitional care • Duration of training: 3 years (shortened HF studies of 2 years possible at apprenticeship qualification as a health EFZ) or 3 to 4 years in part-time training • Own learning transfer training (LTT), including regular exchange with the HF students from the cooperation companies Requirements • Completed apprenticeship as a specialist Health EFZ, Matura or equivalent • Good knowledge of German in word and writing (at least level C1) • Contemporary IT skills (MS Office, Perigon) • High level of social, methodological and professional competence • A sense of responsibility and independent working methods We offer: We offer you a responsible and interesting job in a varied, future-oriented and interprofessional environment.
