For the Senevita Residence Northern Lights in Oerlikon we are looking for a Dipl. Care specialist HF 90-100%, with additional function RAI HC Responsible Your tasks

• You carry out needs assessments with RAI-HC at the customer
• With our customers you take on assignments in basic and treatment care
• You advise and accompany our customers and their relatives
• You carry out RAI assessments and BMF on schedule
• You create care documentation and the individual care planning
• Professional support and support of the Spitex team
• Planning, coordination and evaluation of care missions

You bring that with you

• Certified nurse HF or equivalent education
• Experience in ambulatory care and good knowledge of RAI HC
• Several years of professional experience in long-term care or Spitex region is advantageous
• You are used to resilient, flexible and independent work
• You are a strong communication and responsible personality with team spirit
