As an independent specialist group, the aftercare and social consultancy of the Cantonal Hospital Winterthur is part of the interdisciplinary treatment team and ensures integrated care with viable and sustainable aftercare solutions. We are looking for a reinforcement and look forward to your application as a case manager / social worker / aftercare and social consultancy.

Tasks

• Share responsibility for effective exit management together with all specialists involved in the care
• Perceiving a central role in follow-up and social counselling in the patient recovery process
• Close cooperation with various professional groups within and outside the KSW
• Development and implementation of solutions for the best possible reintegration into society, family and professional life
• Active networking and coordination with internal and external bodies
• Work in an appreciative, positive environment with high recognition of your work

Profile

• Completed training at a college of applied sciences for social work or further education in case management. Basic medical training rounds off your profile
• Initiative, flexible and resilient personality
• Enjoying interdisciplinary cooperation as well as cooperation with external advisory centres and institutions
• Strong communication skills and high commitment to optimal solutions for patients and relatives
• Very good consulting skills and safe handling of different interests and expectations
• Professional experience in an acute hospital, in inpatient rehabilitation or in a comparable complex environment
