Policlinic, emergency, outpatient care, hospitalizations: from birth to old age, the Morges Hospital offers a wide range of care services. The establishment has 240 acute care beds, one maternity, with about 1,500 births per year, and nearly 30,000 emergency consultations. In all, more than 40 medical disciplines have been practiced in its walls for more than 150 years. We are looking for the Hospital of Morges, a: Chronic patient care coordinator and complex F/H situations at 80% to 100% YOUR MISSION In collaboration with family physicians, you independently monitor and coordinate patients with chronic diseases or with complex medical-psycho-social situations. With your sense of organization and initiative, you anticipate the needs of patients and actively contribute to improving their care path. You play a central role in the organisation of this journey to ensure the continuity, quality and economic efficiency of the services. WE OFFER YOU

• A CRS-recognized nursing degree or a Federal Certificate of Ambulatory Medicine Coordinator Clinical Orientation.
• An ability to juggle multiple people and situations while maintaining a patient-centred approach.
• An excellent organizational sense to effectively prioritize and manage an important patient portfolio.
• The ability to work independently while implementing an interprofessional approach.
• A good mastery of computer tools and clinical documentation.
• A good knowledge of the health network and medical and social actors.
• Excellent relational and communication skills. YOUR OBJECTIVES
• Coordinate and support patients with chronic diseases or complex medical-psycho-social situations, in an autonomous manner and in collaboration with family physicians.
• Assess patient needs and identify medical, psychosocial, organizational and economic issues.
• Coordinate interventions between the various professionals involved (doctors, nurses, therapists, social services, caregivers, etc.).
• Ensure regular patient monitoring, anticipate risk situations and facilitate access to appropriate resources and services.
• Develop a care partnership with patients to strengthen their autonomy, active participation in decisions and adherence to treatments and recommendations.
• Participate in the continuous improvement of processes, quality of care pathways and ensure rigorous documentation of interventions. WE ARE
