• Carry out the nutritional assessment at patient entry within the required time frame
• Ensure personalized dietary monitoring according to identified needs
• Evaluate the nutritional status and make an appropriate diagnosis
• Determine nutritional needs and adapt care
• Implement nutritional interventions (oral, enteral or parenteral nutrition)
• Adapt dietary recommendations according to the patient's pathology and preferences
• Evaluate the effectiveness of nutritional care
• Ensure traceability in patient files Qualifications
• Holder of an ES or HES dietician diploma
• Experience in individual consultation, ideally in a clinical or rehabilitation environment
• Experience or marked interest in the nutritional care of the elderly (polypathologies, malnutrition)
• Good command of French, both spoken and written, and ease with current IT tools
• Pronounced taste for working in an interdisciplinary team
• Sense of responsibility, of the organization and priorities
