From January 1, 2027, dementia counselling in Diessenhofen will be conducted under the umbrella of the new specialist centre for age and dementia. With a consulting service you are where people need support: at their home, in a familiar environment or in the specialist office. You accompany affected persons and their relatives individually, strengthen their resources and help to find suitable solutions for everyday life together.

You take over

• Counselling and support for people with dementia and their relatives
• Technical support and orientation in different phases of dementia
• Clarification of individual needs and development of suitable support offers
• Information and advice on relief, support and care opportunities
• Networking and cooperation with relatives, family doctors, Spitex, institutions and other specialists
• Coordination and mediation of suitable services and offers
• Promoting self-employment, quality of life and social participation of the people concerned
• Cooperation in the establishment and further development of the new specialist centre for age and dementia

You bring with you

• Diploma as nursing specialist HF or FH
• In-depth expertise and practical experience in dealing with people with dementia
• Experience in counselling, support and family work is beneficial
• High social skills and an appreciative and empathetic attitude
• Independent, reliable and solution-oriented working method
• Enjoyment of interdisciplinary cooperation and networking
• Willingness to actively shape and further develop a new specialist position
• Good communication and consulting skills
• Own car and driver's license Cat. B is mandatory

The Ambulante Adult Psychiatry (AEP) together with the Psychiatric Clinic Münsterlingen (PKM) is responsible for the psychiatric and psychotherapeutic care of the adult population in Thurgau.
