- Coordination and organization of complex liver treatments (including visits and documentation)
- Care for chronically ill patients (including training on nutrition and muscle building, medication review, vaccination status, social case management)
- Standardized care before and after liver transplantation, including communication with transplant centers
- Co-care for patients with liver cancer (coordination of tumor board, follow-up, interdisciplinary communication)
- Co-care for patients with rare liver diseases and in the transition from pediatric to adult medicine
- Collection of practical knowledge and skills in dealing with chronic liver diseases
