Role overview
As a non-profit organization, Spitex Zimmerberg AG is responsible for the care, advice, support and support of people of different age groups who are dependent on external help due to health restrictions.

In order to support our teams, we are mainly looking for our evening service with assignments in the day service as required by immediately: Care professional HF/FH 50-60 % Your tasks
- InterRAI-HC needs assessments
- Independent, self-responsible care and support of customers
- Taking over the case management
- Improvement of care needs and care costs
- Creating, adapting and evaluating the maintenance documentation according to interRAI-HC
- Carry out and document intervention according to care planning
- Consultation services
- Ongoing review of quality and effectiveness of care
- Cooperation with relatives, doctors: inside, hospitals, homes and other specialists Requirement profile
- Completed training (HF/FH)
