Role overview
The specific field of the nurse is that of the hospitalized imad (known or future clients) who require a return home with imad services.

- Informer clients or future clients of the home services offered by imad, partners of the social health network Genevois, rates and terms of reimbursement of home care benefits.

- Bach HES-SO in nursing or training deemed equivalent and registered by the Swiss Red Cross
- Knowledge of the clinical care process and routes, the social health network Genevois
Main responsibilities
- Informer clients or future clients of the home services offered by imad, partners of the social health network Genevois, rates and terms of reimbursement of home care benefits.
- As necessary, direct the client to the person or structure who can advise him.
- Ensure coordination between imad and the private/public hospital sector to arrange the return home of clients, with services provided by imad.
- To facilitate the discharge of clients' hospitals and to facilitate the return to home in the best conditions.
- To work with the client, the carer, health and social professionals.
- To prevent inappropriate hospitalizations.
- To work closely with the team of the Imad arcade located at the HUG.
