- They accompany the exit planning for defined patient groups as part of the need clarification, advice and organization of the necessary measures related to the post-stationary care of patients in consultation with the interdisciplinary team You clarify social claims for our patients
- They advise and support patients and relatives with the aim of promoting social reintegration in terms of treatment, future and help planning
- They advise our patients on psychosocial and social insurance issues arising from hospital stays and provide support for optimal networking
- They work with external service providers as well as the service providers in the sense of good cooperation and networking
- They support the search for transition care, nursing homes and hospices
- They establish contacts with consultancy offices and self-help groups.

- Professional experience in health care in Switzerland, preferably in an acute hospital or advice, and are interested in addressing the above-mentioned challenges
