To complement our team, we look for you immediately or by agreement Dipl. HF nurse (pensum 80–100 %) Their tasks
- Day-to-day responsibility and first point of contact for our customers, our employees as well as for doctors and hospitals
- Need-, situation- and resource-oriented care, support and advice of our customers at home, taking into account the social environment
- Determination of care needs by means of inter RAI HC and creation of care planning
- Evaluation of care planning for customers
- Cooperation with relatives, doctors, hospitals, homes and other specialist centres
- Client support in somatic and psychiatric care

- Experience in ambulatory care or interest in obtaining it with us
- Basic course RAI HC desirable
