Tasks

You help shape the path of our patients. Do you think in a networked way, keep an overview even in demanding situations and want to actively shape the stay of our patients? Then you are exactly right with us. As a patient manager, you coordinate the treatment process from entry to exit. Together with medicine, care, therapy and external partners, you ensure that the right follow-up solution is available at the right time. In this way, you make an important contribution to safe and efficient patient care.

• You coordinate the patient process from entry to exit supports forward-looking exit planning
• You plan and organize the follow-up care together with all participating professional groups
• You will advise patients and relatives on the various follow-up solutions
• You organize rehabilitation, nursing home entrances, and other follow-up care
• You are in direct contact with insurers, especially for cost-benefit languages and renewal applications
• You work closely with the treatment teams and external institutions Optional: Deputy leadership function

Do you want to take on more responsibility and shape the future of patient management? Then this function offers you the opportunity to take on the deputy management function at the location. You support the management of the central patient management in everyday operations, represent them in absences and actively contribute to the further development of our processes and cooperation.

Requirements

• Education and experience in health care as a Dipl. Nursing professional HF, ideally with further training in case/care management
• Counselling and socially competent personality with sovereign, heartfelt appearance
• Organizational talent, initiative and enjoyment of interdisciplinary cooperation
• A solution-oriented, independent and communicative working method
• You keep an overview even in demanding situations and convince with an independent, solution-oriented working method
