Company and context
- Recognizing, triaging and accompanying acute crisis situations (including organization of home visits and other interventions)
- Families and treatment teams are networked and advised using local resources (paediatricians, children's hospitals, therapeutic services and institutions); the flow of information throughout the entire care network is ensured
- Planned hospitalizations are prepared and transitions

- Individual care plans are drawn up and implemented together with those involved
