In dieser Funktion übernehmen Sie eine zentrale Rolle in der Betreuung unserer Patientinnen und Patienten und arbeiten eng mit dem ärztlichen Dienst sowie weiteren Berufsgruppen zusammen.

- Sie unterstützen im interprofessionellen Team die Ärzteteams auf den Bettenstationen des Innere Medizin.
- Sie betreuen Patientinnen und Patienten über das gesamte Abklärungs- und Behandlungskontinuum und arbeiten dabei eng mit den Kaderärztinnen und Kaderärzten zusammen.
- Sie verfügen über einen Abschluss als MScN, BScN oder MAS/DAS Physician Associate.
- Sie konnten bereits Berufserfahrung als Klinische/r Fachspezialist/in sammeln.
Main responsibilities
- Sie unterstützen im interprofessionellen Team die Ärzteteams auf den Bettenstationen des Innere Medizin.
- Sie betreuen Patientinnen und Patienten über das gesamte Abklärungs- und Behandlungskontinuum und arbeiten dabei eng mit den Kaderärztinnen und Kaderärzten zusammen.
- Sie übernehmen klinische, organisatorische und administrative Aufgaben in Innovationsprojekten wie Hospital at Home und Geriatrisches Co-Management.
- Sie wirken bei Aufgaben im Rahmen der klinischen Begleitforschung mit.
- Sie koordinieren die Care Transition im Spital sowie den Übergang vom stationären in den ambulanten Bereich.
- Sie führen prä- und poststationäre Abklärungen durch und beurteilen im interprofessionellen Team, ob weiterführende Massnahmen erforderlich sind.

- Sie verfügen über einen Abschluss als MScN, BScN oder MAS/DAS Physician Associate.
- Sie konnten bereits Berufserfahrung als Klinische/r Fachspezialist/in sammeln.
- Sie sind eine engagierte und teamorientierte Persönlichkeit.
- Sie arbeiten selbstständig und behalten auch in anspruchsvollen Situationen den Überblick.
