• You treat and advise your patients as well as their relatives holistically, professionally and individually.
• You ensure the quality and continuity of care process management.
• You implement individual care planning together with the interprofessional treatment team.
• You recognize changing situations at an early stage and initiate the appropriate measures.
• You support smooth shift operation and cross-departmental cooperation.
• You are actively working on the further development of the care areas. Your profile:
• You have a degree as Dipl. Career Professional HF/FH (Dutch Diploma: SRK Recognition)
• Professional experience in acute care and in the field of cardiology and nephrology is desirable
