Role overview

Tasks

• Advise, care for and support people with a serious progressive illness in their home environment
• Independent assessment of unstable and complex care situations
• Treatment of complaints (symptom management)
• Independent planning and organizing of missions
• Establish a sustainable care team at home and advise members and relatives
• Coordinating cooperation with external specialist bodies and specialists
• Conducting evaluations and adapting electronic documentation
• Participation in quality assurance and further development of care processes
• Participation in the Mobile Palliative Care Team Requirements
• Several years of experience as a nurse HF/FH
• Training in Palliative Care / Oncology Care Level B2 or CAS
• High professional and social competence as well as a good self-assessment
• Appreciative communication
• Appropriate IT user knowledge
• Willingness to travel with the Velo
