In order to strengthen our Medical and Nurse Clinical Practitioner (NCP) team in the Department of Internal Medicine, we are looking for a dedicated personality by appointment or immediately. In this function, you play a central role in the care of our patients and work closely with the medical service and other professional groups. They accompany patients throughout the entire processing and treatment continuum and bring their clinical expertise into innovative supply models. Your tasks:

• In the interprofessional team, they support the medical teams at the beds of the department of Internal Medicine.
• Together with or delegated by cader doctors and cader physicians, they provide care for patients throughout the entire clarification and treatment continuum.
• They perform clinical, organizational and administrative tasks in innovation projects such as Hospital at Home and Geriatric Co-Management.
• They contribute to tasks in the context of clinical accompanying research.
• They coordinate the Care Transition in the hospital and the transition from the stationary to the outpatient area.
• They conduct pre- and post-stationary clarifications and assess in the interprofessional team whether further measures are necessary. Your profile:
• You have a degree as MScN, BScN or MAS/DAS Physician Associate.
• They are ideally already bringing professional experience as a clinical specialist or Advanced Practice Nurse (APN). You have a category B driver ID.
