- Independent, responsible, and structured way of working Good administrative and IT skills
Company and context
- Mediation and organization of doctor-prescribed rehabilitation and cure stays
- Support, advice, and organization of transfers to nursing and retirement homes
- Advice and support in planning ambulatory care at home (e.g., Spitex, meal services, etc.)
- Mediation and advice for institutions and offers in the field of social services, addiction help, and disability facilities
- Coordination with relevant external institutions, authorities, and municipalities
- Participation in interdisciplinary reports, visits, working groups, and projects
- Planning and organization of patient transports
- Recording of patient statistics in one's own area of responsibility Requirements:
- Specific training and further education in the field of social work/healthcare; relevant professional experience in a comparable institution of the healthcare or social sector is an advantage
- Good knowledge of relevant issues in the field of health, accident, and social insurance law
