Do you want to help people with dementia to live safely and self-determinedly in their familiar home for as long as possible? In the Memory Care Team at Spitex Zurich, you will accompany customers with cognitive limitations holistically – from needs assessment to care and advice for relatives. You take on case responsibility and design individual solutions that make everyday life easier for those affected and their environment. Your assignments take place during the day – usually between 07:00 and 16:00.

YOUR TASKS

• You take over the case management for people with dementia and control the care process from needs assessment to planning to the regular evaluation of the agreed measures.
• You nurture and look after our customers in their familiar home, promote their existing resources and help them to maintain their independence as long as possible.
• You advise and accompany relatives, coordinate cooperation with doctors and specialists and are an important contact person for all parties involved.
• You work closely with colleagues from different professional groups as part of the Skill & Grade Mix and ensure careful and seamless care documentation.
• You network with internal and external specialists, contribute your knowledge to the team and support new employees, learners and students in their training.

You bring with you

• Completed tertiary level nursing training
• Friendly and appreciative appearance as well as physical and psychological resilience
• Joy of change and cooperation in the team
• High sense of responsibility, independence and flexibility
• Good knowledge of the German language in word and writing, mind. Level B2
• Driving licence Cat. B or willingness to travel with the electric vehicle
