- You control care processes, coordinate demanding situations and together with the team ensure high-quality care for our clients.
- You work independently, with a lot of creative leeway and in close cooperation with internal and external specialists.
- Your main tasks include: Assumption of the case management including demand assessment by means of interRAI HC as well as planning, implementation and evaluation of the care processes Ensuring high-quality, client-oriented care in the home environment Interprofessional cooperation with family doctors: inside, relatives, institutions and other specialists Professional support and support of your teammate: women in complex care situations Careful documentation and active participation in quality assurance
- Requirements Completed training as a dipl.
- Care professional HF/FH At least one year of professional experience, ideally in the Spitex environment Enjoy independent and responsible work Good IT user knowledge (digital maintenance documentation) Driver's license Cat.
