Nurse HF/FH with case management (80-100%) | Spitex As a qualified nurse with case management, you take on a central role with us. You control care processes, coordinate demanding care situations and together with your team ensure professional and high-quality care for our clients in their familiar home. You work independently, bring your expertise specifically and get the necessary creative scope to actively shape care processes.

Your main tasks include

• Assumption of the case management including demand assessment by means of interRAI HC as well as planning, implementation and evaluation of the care processes
• Ensuring professional, individual and client-oriented care in the home environment
• Coordination and interprofessional cooperation with family doctors: inside, relatives, institutions and other specialists
• Professional support and support of your teammate: women in complex care situations
• Professional care documentation and active participation in quality assurance and development

This is what you bring with you

• Diploma as a nurse HF/FH or an equivalent education recognised in Switzerland
• professional experience as a qualified nurse
• Experience in the Spitex environment is an advantage
• Enjoy independent and responsible work as well as the coordination of complex care situations
• Organizational skills, communication skills and enjoyment of interprofessional cooperation
• Good IT user knowledge and openness to digital tools
• Driver's license Cat. B or willingness to visit our clients: inside in any weather with the e-bike
