Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our offer ranges from counselling for older people and their relatives to general and specialized long-term care and care at home. As the largest public Spitex in the Dielsdorf district, we provide ambulatory services for our nine contract municipalities. With our comprehensive range of care services, we enable people to stay in their usual home for as long as possible despite health restrictions. In the future, it is planned to establish a night spitex.
Tasks: • Care, support and advice of our clients at home • Classification and design of your work during the day from Monday to Friday • Responsibility for carrying out investigations with Inter RAI-CMH, the psychosocial care and care of chronically and mentally ill people as well as conducting conversations and recovery-oriented support of therapeutic processes • Intensive exchange, also beyond the borders of the Dielsdorf health centre, with relatives, social workers: the KESB, specialists, doctors, etc. • Active participation in the development and development of the psychiatry spitex Requirements • Completed training as a Dipl.
