- You actively shape the development of our palliative care team from the beginning.
- Together with the care expert and the management, you will develop structures, processes and professional standards for specialized outpatient palliative care.
- You accompany clients with complex palliative needs as well as their relatives in their home.
- You take on the professional responsibility for demanding palliative care situations and coordinate the care process.
- You work closely with the needs assessment team, primary care physicians, specialized palliative care services, hospitals, hospices, therapists and other network partners.
- You bring your expertise to the organization, support the development of knowledge and accompany employees with palliative questions.
