- Reference personnel work and nursing case management: You plan, control and evaluate the care process of the patients assigned to you.

- Nurse HF/FH; for foreign diploma with corresponding SRC recognition
Main responsibilities
- Your responsibilities
- Reference personnel work and nursing case management: You plan, control and evaluate the care process of the patients assigned to you.
- Shift responsibility: You coordinate the shift professionally, set priorities and ensure a clear distribution of tasks and delegation within the care team.
- Psychiatric care: You assess the mental state, recognize changes at an early stage and act professionally and appropriately in crises and challenging situations.
- Old-age psychiatric care: They accompany people with different clinical conditions such as dementia, delirium, depression, dependency diseases or psychosis as well as psychiatric and somatic multimorbidity.
- Family work: They advise and accompany relatives and involve them appropriately in the treatment process.
- Interprofessional collaboration: You bring your nursing expertise to visits and case discussions and actively participate in setting and reviewing treatment goals.
- Exit and transition management: Together with the treatment team, you design viable follow-up solutions, working with Spitex, homes, referees and other external partners.

Key requirements
- Completed training as a Dipl.
- Nurse HF/FH; for foreign diploma with corresponding SRC recognition
- Professional experience in the psychiatric field desired
- Willingness for professional development (including full cost coverage of an age-relevant CAS by the Hospital Affoltern)
