Participation

• Ensure efficient, needs-based and sustainable exit and transfer planning with the aim of ensuring quality of care and avoiding unnecessary re-entry and rehospitalisation
• Holistic case management of patients during the inpatient stay, taking into account the individual social, medical and care-related situation
• Conducting and coordinating interdisciplinary discussions with patients, relatives and internal and external specialists
• Planning, organization and coordination of transfers in rehabilitations, cures, care institutions or care at home
• Networking and cooperation with internal and external service providers, public authorities, insurance companies and other bodies involved

Bring with you

• Education in health, social or related fields, ideally with further training in case management
• Professional experience in health care as well as experience in interprofessional cooperation
• Experience in advising and accompanying people in stressful life situations
• Strong communication, coordination and organisational capabilities
• Independent, solution-oriented and networked working method
• Knowledge of social security
• Enjoy working with patients, relatives and internal and external specialists
