Main responsibilities
- You are interested and motivated
- to be on the go independently, but to be part of the treatment team
- to accompany and support clients in difficult situations as a reference-career (or case-leading)
- to exchange information as a reference person with external partners, such as domestic doctors, the PallCare team, nutrition advice, employees in hospitals and homes
- solution-oriented, based on the care process, to work
- to electronically record the care documentation directly with the customers
- to work together and exchange various professional groups in an interdisciplinary team
- to engage in case meetings and to reflect on their own as well as the work of others digitally exchange ideas with colleagues
- to move in different roles in everyday work - as a model, as a reference person, as a daily officer, as a specialist of Spitex, etc.
- to understand themselves as carers in our customers and their relatives
