Reference Nurse (M/F)

Participation rate between 80% and 100% Undetermined fixed-term contract - daytime and a few weekends

Your Mission

• Support clients in their home living project with a partnership perspective by giving meaning and coherence in close collaboration with the surrounding community and family, while building trust
• Pilot the overall and integrative care of clients while relying on in-house stakeholders and interdisciplinary teams and in good coordination with the institutions involved in the care network
• Develop the therapeutic project taking into account the complexity of care and develop it according to evolution
• Providing assessment, counselling, examination and treatment, as well as nursing and basic care services for quality client care
• Actively contribute to the sustainable home care of clients through prevention and health promotion measures while taking into account individual needs with personalized care and promoting client autonomy
• Participation in development projects and working groups on continuous improvement of benefits in the field of home care and care

Your profile and skills

• Bachelor's degree in nursing or equivalent (foreign diploma recognized by the CRS)
• Medical and social experience and strong interest in community health
• Autonomy and decision-making skills
• Ability to collaborate in networking and multidisciplinarity
• Responsibility and professional ethics
