Nurse Coordinator (M/F)

80% participation rate Undetermined fixed-term contract Your missions

• Identify, analyze the immediate needs of the client and establish the first intervention plan to optimize continuity of care.
• Secure clients' return home and coordinate care during care.
• Provide care during care time.
• Provide telephone daycare on nights, weekends and public holidays.
• Collaborate with and respond to client network stakeholders.
• Respond to acute situations at home as needed.

Your profile and skills

• Bachelor's degree in nursing or equivalent (foreign diploma recognized by the CRS)
• Experience in the medical-social field and in interdisciplinary collaboration
• Knowledge in the care networks of the canton of Vaud
• Responsibility, priorities and autonomy
• Essential driving licence and desirable vehicle
