The Riviera-Chablais Hospital, Vaud-Valais (HRC), is a reference intercantonal hospital with over 2,000 collaborators. Rennaz Hospital has over 350 acute care beds. It is complemented by two Geriatric and Rehabilitation Clinics and two medical clinics. All employees invest on a daily basis to provide quality care for patients, embodying the institution's values of authenticity, exemplaryness and innovation.

Its Geriatric and Rehabilitation Service SGR offers services on 3 sites, namely:

• The Clinic of Geriatrics and Rehabilitation CGR of the Riviera in Vevey (site of Providence)
• The Geriatric Clinic and Rehabilitation CGR of Chablais in Monthey
• Rennaz's website.

The RMC's mission is mainly geriatric rehabilitation, as well as acute care for seniors. In Rennaz they are consultations of Hospital Geriatrics (HCG).

A close collaboration with the HRC allows our patients to benefit from its technical platform and latest generation equipment.

Under the hierarchy of a medical-care binoma, you will work in close partnership with the patient and his or her relatives, while focusing on interdisciplinary collaboration.

The Service of Geriatrics and Rehabilitation seeks

Community Health and Care Assistants (CHS) at 80%-100%

The main tasks of this post are

• Organize, execute, monitor and document medical and technical services delegated by the nurse and authorized by the ASSC status, in stable situations of no emergency character
• Collaborate in the smooth operation of the service
• Promote the interdisciplinary approach to care and team work specific to the mission of the service
• Promote, by delegation, the patient's involvement in his or her care project and that of his or her relatives

• Imperatively an SWC in Community Health and Care
• A person motivated to take care of seniors according to the good practices of Geriatrics and Rehabilitation
• A rigorous, organized and dynamic person with excellent communication skills
• An interest in working as an interdisciplinary team
• Adaptation to change and involvement in the service project
• A mastery of the French language and computer tools specific to the hospital field
