Tasks
- You accompany people when entering, leaving or changing within the age center – and design these transitions with heart, organizational talent and vision, always with a focus on the needs of the residents and their relatives as well as in close cooperation with care, administration and external partners.
- You advise residents and relatives individually and empathetically as part of a case management
- You organize visits and information discussions with (potential) residents and their relatives
- You lead the bed mirror according to business criteria and ensure a forward-looking and needs-based room occupancy
- You support with applications for supplementary services, helpless compensation and connection solutions
- You maintain contact with institutions such as KESB, AHV, Pro Senectute and other relevant bodies
Requirements
- Completed nursing or medical training with a tertiary degree – ideally supplemented by further training in case management
- Empathy, communication strength and the ability to act clearly and solution-oriented even in challenging situations
- Good IT user knowledge and a networked, entrepreneurial thinking
- High flexibility and resilience in a dynamic environment
