We are looking for a

Social Worker for Home Socio-Educational Follow-ups (60 to 80%) Immediate start or to be agreed

• secure client support at their home in daily tasks and activities according to the institutional rehabilitation concept, staff regulations, and safety standards;
• development documented individual projects with your referees, ensuring the interdisciplinarity of the projects. Guarantee their follow-up;
• manage the individualized follow-up file of your references;

Your profile

• you hold a diploma as a social worker HES / ES or an equivalent qualification. A CAS in psychiatric is an asset;
• you have confirmed work experience in psychiatric or social work in a residential home and/or home follow-up;
• with a very good sense of organization, you enjoy working independently and in a team and can take on responsibilities;
• you are professional with common IT tools such as MS Office;
• holder of a driving license, you have a private vehicle.
