RAI Nurse - HC (M/F)

Participation rate between 80% Fixed contract with undetermined duration

Your Mission

• Conduct assessments and reassessments of all client situations at home using the Resident Assessment Instrument (RAI-HC) method
• Assess situations and identify needs
• Plan, coordinate, provide and monitor care
• Advise and inform cared persons and their surroundings
• Contributing to health maintenance and disease prevention through an interdisciplinary approach
• Enhancing personal autonomy and adaptability
• Developing the quality of care and adapting services to new knowledge

Your profile and skills

• Bachelor's degree in nursing or equivalent (foreign diploma recognized by the CRS)
• RAI-HC Evaluator Training an asset
• 2 years of proven experience in the medical and social field, in interdisciplinary collaboration and networks
• A sense of responsibility, ability to organize independently and set priorities
• Ability to collaborate in networking and multidisciplinarity
