The CNP aims to rehabilitate patients suffering from mental disorders through the adoption of an integrated and community psychiatry model; it implements public policies for the promotion and prevention of mental health; it acts as a reference center in the field of professional training and research in psychiatry. • Mobile Team 80-100% s/ref: 2026072 As a Nurse Case Manager within the Littoral Mobile Team, you support patients suffering from psychological disorders with a view to recovery and psychosocial rehabilitation. You guarantee the continuity, coordination and quality of monitoring, particularly during periods of transition and instability. As part of your role you will be required to:
• Identify, in collaboration with the outpatient site, hospital and psychiatric emergency teams, patients for whom case management transition monitoring is indicated.
• Carry out an overall assessment of patient needs, by consulting the various stakeholders and establishing an individualized therapeutic plan.
• Maintain a directory of potentially eligible patients, and contribute to the development and revision of case management monitoring indicators.
• Ensure regular and flexible monitoring, coordinate interventions, and support liaison in all structures involved (accommodation, home, hospitalization, outpatient).
• Strengthen collaboration with partners in the Neuchâtel network (FADS, SPNE, RHNe, Addiction Neuchâtel, SMIG, Maison de Santé, Medic Help NE, Association Route, Médecins du Jura, associations, etc.), in particular through regular contact and meetings.
• Conduct individual, family and network interviews, and participate in leading therapeutic groups.
• Provide a specialized third-party perspective and clinical support to hospital or supervisory teams from partner structures in complex situations.
• Collaborate in the monitoring, evaluation and development of the case management project within the adult department.
• Establish a relationship of trust with the patient, their loved ones and the multidisciplinary team, promoting the participation of all at the stages of the care process.
• Requisiti The profile we are looking for is as follows:
• Hold a Bachelor's degree in nursing or a title deemed equivalent and recognized by the Swiss Red Cross.
• Training and experience in case management is an asset for the role.
• Have at least 3 years of experience in psychiatry.
• Be in possession of a driving license.
• Have knowledge of therapeutic measures.
• Know psychiatric illnesses and their impact on social and daily life.
• Knowledge of mental health, psychosocial rehabilitation techniques.
