Role overview
You take over assessment of the current social situation of patients and definition of social problem areas in cooperation with the multi-professional team and the affected persons Advice and support for patients and their relatives in matters of work, finance and housing, as well as social insurance matters Keeping contacts with authorities and post-care institutions Cooperation in exit management and the organisation of post-stationary care and/or new residential forms Participation in multi-professional reports, team meetings and supervisions Coordination of the social-working tasks, including the entire patient environment and the multi-professional team A completed training as a social worker/in FH professional experience in the psychiatric or medical context is of advantage Good knowledge of the Swiss social system Basic understanding, empathy and communication skills in dealing with mentally ill people in the higher age of life and their relatives Flexibility, self-employment, resilience and team skills 1007000|1007002|1007004|1007006|1007008|1007010|20000124|20000126|20000128|20000130|20000132|20000134 Area: Social occupations Type: Parttime Deadline: Unlimited
