- Dipl.
- Care specialist Spitex with case management (70–100%)
- Responsibility for case management including conducting clarification and consulting discussions with InterRAI-HC clarifications
- planning, implementation and evaluation of the care processes
- ensuring the need-based and client-oriented care in the home environment as well as adaptations to changed situations
- Interprofessionelle Zusammenarbeit mit Hausärzten, Beziehmen, Institutionen und weitere Fachpersonen
- Active participation in the quality assurance and further development of the department Somatik Requirements
- Completed training as a nursing specialist HF/FH as well as experience in case management and care process control (RAI-HC)
- Well-founded expertise in the care and care of people in complex situations in outpatients in the 1970s
