In cooperation with a second Nurse Practitioner (NP), you and the USZ@Home Team support patients who are followed up at home after leaving the hospital.
In the development phase, the patient is: inside with IV therapies, wound dressings or pain.

- Participation in the development and implementation of the new USZ@Home care model, e.g. elaboration of treatment pathways with the participating clinics, creation & implementation of guidelines for the care of USZ@Home patients: inside, establishment of a stable cooperation with external partners such as Spitex and GP
- Assessment and inclusion of suitable patients: insiders for USZ@Home care
Main responsibilities
- Participation in the development and implementation of the new USZ@Home care model, e.g. elaboration of treatment pathways with the participating clinics, creation & implementation of guidelines for the care of USZ@Home patients: inside, establishment of a stable cooperation with external partners such as Spitex and GP
- Assessment and inclusion of suitable patients: insiders for USZ@Home care
- Carrying out telemedical, interprofessional visits Exit management from the USZ@Home Setting
- Coordination of various internal and external services
- Coaching & empowerment of internal and external employees to implement the USZ@Home treatment pathways
- Further development of the USZ@Home care model together with the project team for scaling for further groups of patients: Requirements
- A qualified nurse with several years of professional experience and a completed Master of Science in Nursing.
- A person with an interest in merging research & practice in everyday working life and is dedicated to the further / development of the USZ@Home care model with commitment and pleasure.
