- Eigenverantwortliche, qualifizierte und ressourcenorientierte Pflege und Beratung unserer Kunden, An- und Zugehörige
- Bedarfsabklärung RAI-HC, steuern und überwachen des Pflegeprozesses
- Abgeschlossene Ausbildung als diplomierte Pflegefachfrau/Pflegefachmann HF, FH mit Fachausbildung Palliative Care/Onkologie Care B2 Spitex-Erfahrung von Vorteil
- Hohe Fach- und Sozialkompetenz, selbständige und verantwortungsbewusste Arbeitsweise

- Eigenverantwortliche, qualifizierte und ressourcenorientierte Pflege und Beratung unserer Kunden, An- und Zugehörige
- Bedarfsabklärung RAI-HC, steuern und überwachen des Pflegeprozesses
- Zusammenarbeit mit Angehörigen und involvierten Fachpersonen
- Pflegt enge interprofessionelle Zusammenarbeit und Koordination mit Fachpersonen verschiedenster Bereiche Ihre Perspektive
