What to expect
- Day service from Monday to Friday incl. Public holidays
- Independent, competent and resource-oriented care of our clients and their relatives in dealing with dementia in their usual living environment
- Assumption of case management for a defined client group
- Planning, control and monitoring of the care process
- Support our clients in a self-determined everyday life at home by promoting independence and quality of life
- Interdisciplinary cooperation with relatives, doctors and other services
- Conducting assessments and site discussions with RAI-HC
