- They ensure a stable and coordinated aftercare after the hospital exit
- They work with innovative projects of integrated care and virtual care Requirements:
- They have completed training as a medical practice assistant / medical practice assistant EFZ or as a specialist / skilled worker Health EFZ and bring professional experience in healthcare with
- They bring experience in the coordination of complex treatment paths and understand how to combine different disciplines, processes and needs patient-oriented
- They enjoy interprofessional cooperation and use digital communication channels securely, efficiently and addressably
- They work structured, independent and with a high sense of responsibility – always with a view to the needs of patients and for smooth processes We offer:
