- Advice for patients with the involvement of relatives during the hospital stay in preparation for leaving, mediation of follow-up solutions
- Interdisciplinary cooperation in the care of seriously ill and dying patients: insiders and their relatives
- Intermediation of external specialist services and medical aids
- Advising patients on insurance and legal issues incl. Clarification of financing issues, development of financial support or under-utilized sources of financing
- Obtaining cost language at home and abroad
- Interdisciplinary cooperation with all nursing stations and the medical and nursing service
- Cooperation and networking with after-care institutions
- Collaboration in hospital projects Your profile
- Completed study Social Work FH or equivalent
- Experience in health care, social service, case management, or basic training in nursing beneficial
- Very good communication skills
- Solution-oriented approach to crises and problem situations
- Strong social skills and empathy
- Willingness to cooperate interdisciplinary
- Learning and interested to complete education/further training
- Professional management of public services
- Motivated and committed personality who is characterized by an independent working method and can make decisions by hand
- Varied, responsible area of responsibility in a small motivated team
