Their tasks
- Organization of the entire exit/transfer procedure and implementation of the necessary working and team meetings
- Advice with experts on financial and social law issues
- Cooperation with authorities for clarification of assistance
- Advice for spa, rehabilitation or nursing stays
- Information on ambulatory offers
- Obtaining cost credits from health insurers for spa contributions and rehabilitation
- Responsibility for the complete documentation of ongoing cases including keeping of statistics
- Networking with counselling centres such as Pro Senectute, Cancer League, Rehab and Social Psychiatric Services
- Cooperation and coordination with doctors, nurses, patients, relatives and external partners
Your profile
- CAS in Case Management or equivalent training in Case Management is beneficial, or willing to complete
- Independent and structured working method
- Motivated, honest, team capable and friendly personality with a high sense of responsibility
- High social and self-competence
- Advantageous, medical expertise or already many years of experience in health care
- Experienced PC user knowledge
- Very good knowledge of German in word and writing
