Application process
- InterRAI HC assessments: Application of the InterRAI HC system to carry out standardized needs assessments to ensure precise care and support planning.
- Advice: Advising clients and their relatives regarding care and care services.
- Creation of nursing plans: Development of individual nursing and care plans in cooperation with the nurses and the interdisciplinary team.
- Coordination: Close cooperation with nurses, doctors, Spitex employees and other specialists to ensure seamless care. Documentation:
